IVF (in vitro fertilisation)

The typical IVF cycle, step by step

Every protocol is individual, and your doctor will decide the details. A typical cycle looks like this:

1. Tests and assessment. Hormone blood tests, an ultrasound of the ovaries and uterus, a semen analysis and infection screening for both partners.
2. Ovarian stimulation. Daily hormone injections, typically for around 10 to 14 days, to help several eggs mature. You are monitored with ultrasound scans and blood tests every few days so the doses can be adjusted.
3. Trigger injection. When the follicles are ready, a final injection prepares the eggs for collection, usually about a day and a half later.
4. Egg retrieval. A short procedure, usually under sedation, in which eggs are collected through the vagina with ultrasound guidance. The partner's sperm sample is usually collected the same day.
5. Fertilisation. Eggs and sperm are combined in the lab by standard IVF or ICSI, and the embryos are observed for several days.
6. Embryo transfer. Typically three to five days after retrieval, an embryo is placed in the uterus with a thin catheter. Sometimes the doctor advises freezing all embryos and transferring later.
7. Pregnancy test. A blood test is typically done about two weeks after the transfer.

Possible risks

IVF is a widely used treatment, but it is not free of risk. The main ones your doctor will discuss include:

• Ovarian hyperstimulation syndrome (OHSS): the ovaries react too strongly to the medication, causing bloating, pain and, rarely, more serious problems. Careful monitoring aims to reduce this risk.
• Multiple pregnancy: transferring more than one embryo increases the chance of twins or more, which carries higher risks for mother and babies.
• Discomfort, minor bleeding or, rarely, infection after egg retrieval, and side effects of the hormones.
